Arranging a Smooth Transition from Hospital to Home

By working very closely with your child’s primary care physician or specialist, our Clinical Supervisors develop an individualized treatment plan for your child’s care, including education and training to assist you in caring for your child at home. The plan is designed to enhance your child’s well-being and enable him/her to reach their full potential.

At your request, we will make ourselves available to actively participate in your child’s pre-discharge conferences. We can help prepare your child – and your family – for the discharge from the hospital and the transition to home care. The success of that transition is also dependent on the appropriate selection of equipment, training, and support personnel. Working with the hospital discharge planner, we help to coordinate everything necessary to ensure a successful homecoming for all.


Assessing the Home Environment

Our clinical supervisors will meet with you at home prior to discharge to help you determine the best way to prepare your home for optimal success in making your child and family comfortable there. We can assess the areas in which your child will be cared for, and ensure there is adequate space to accommodate necessary equipment. We can work with your extended team to ensure that all aspects of care, such as equipment and supplies, are available to you.


Support for Your Family

Our pediatric nurses understand that, with a medically fragile child at home, the whole family requires physical and emotional support. It can be difficult to have a non-family member in your home, especially for extended periods of time. For this reason, we train our pediatric nurses to be sensitive to your family’s unique situation, and establish in-home protocols that minimize disruptions to your family’s routines. We also have team members who have parented children with medical complexities who can provide support and help you navigate the challenges that come into play when you have a medically fragile child at home.


Billing and Insurance

Caring for a child at home can be expensive. We help families access the appropriate resources to minimize any financial hardship. We can assist with private insurance policies, Medicaid eligibility including SSI and Medicaid Waiver programs, management of insurance verification, billing, and submission of required paperwork.
